Present Study on Non Ulcer Dyspepsia in Chinese Mainland

Abstract

In chinese mainland, non ulcer dyspepsia( HUD) is a common functional disease related with dysmotility in upper GI. tract. It is about 20-40% outpatients in digestive clinic and 30-60% gastroendoscopic patients. Since 1994, five research centers in Beijing, Shanghai. Guanzhuo, Wuhan and Xi an have been founded. Two or four regional seminars on GI motility have been held every year. In analysis of 8896 NUD from 500 hospitals in 20 provinces and cities. The main clinical manifestations were epigastric distention( 89.7%), postprandial fullness(87.7%), belching(74.7), esrly satiety(63.2%), epigastric pain(57.5%) , anorexia(52.8%) and vomiting(50.0%). NUD was classified as dysmotility(61.8%), ulcer-like(17.5%), mixed type(10.0%) and reflux-like(7.9%). The etiology of NUD is uncertain. By means of double nuclides labelled meal, delayed gastric emptying was found in 60-70% NUD especially for solid food. With B ultrasound and gastroelectrography, distubance of antral-duodenal motility and gastroelectric rhythm was demonstrated. There was no significant relation between HP infection and NUD. But in 30-40% NUD eradicated HP, most of symptoms relieved or disappered. In diagnosis, the following should be emphosized: (1)Dyspeptic symptoms most persist more than one month. (2)Systemic organic diseases should be excluded with X-ray, beta ultrasound, endoscopy and laboratory tests. (3)by gastroendoscopy and pathology, only superficial gastritis could be found, without erosion and atrophy. For treatment, we should pay attention to: (1)Psycho1ogical treatment is very important. If necessary, antianxiety drug Alprazolam or deanxit shoule be chosen. (2)It is necessary to treat NUD with Prokonetic drug cisapride(88.8%)for at least one month. (3)In mainland, gastric cancer is the first dead cause. I believe that HP eradication should be necessary. (4)NUD is similar to Chinese medicine termed a lump in the abdomen. ZHishi xiaopi wan is effective for 81.49% NUD.
